Analysis of sperm oxidative stress (ROS test)

One of the factors that can reduce male fertility is the hyperproduction of reactive oxygen species (ROS), which lead to the development of oxidative stress in sperm and, as a result, male infertility.

When reactive oxygen species are present in excess in semen, they can initiate pathological changes in sperm cells, causing oxidative damage to cell membranes, proteins, and DNA.

Such disorders are called sperm oxidative stress.
Oxidative stress is a condition in which reactive oxygen species have a toxic effect due to their increased formation or as a result of impaired antioxidant defense mechanisms.

Risk factors for increased levels of reactive oxygen species in semen (sperm oxidative stress)

  • infectious and inflammatory processes in the organs of the genitourinary tract
  • varicocele
  • cryptorchidism
  • overheating in feverish states, or exposure to external heat sources (sauna, hot tub, etc.)
  • psycho-emotional stress;
  • autoimmune reactions against spermatozoa, accompanied by the production of antisperm antibodies (ASAT)
  • systemic diseases (diabetes, gout, etc.)
  • smoking
  • age over 40 years old
  • lack of antioxidants in food
  • genetic defects in the antioxidant defense system.

Oxidative stress of sperm occurs in 30-80% of cases in couples who cannot get pregnant. This study helps to identify such a factor in male infertility.
The results of this test, along with other studies conducted in the clinic, are used to select the optimal tactics for restoring a man's fertility and choosing the most effective assisted reproductive technology method.

Mechanisms of impaired fertility in men in the development of oxidative stress:

  • Reduced sperm motility: reactive oxygen species reduce the flexibility of the germ cell membrane and, consequently, the movement of the tail;
  • direct oxidative damage to mitochondria reduces their ability to produce energy, which in turn contributes to a decrease in sperm motility;
  • Reduced ability of the sperm to penetrate into the egg: reactive oxygen species lead to peroxidation of the acrosomal region of the sperm-egg membrane;
  • reactive oxygen species can directly damage sperm DNA, (fragment) leading to the transmission of a defective paternal genome.

The potential reduction of reactive oxygen species and oxidative stress can be achieved through diet and lifestyle changes. Diet and antioxidant intake also help to strengthen the antioxidant system

The most affordable antioxidants:

  • alpha-lipoic acid
  • vitamin A, E, C and beta-carotene
  • cysteine
  • zinc
  • flavonoids
  • glutathione

Preparation and/or course of the procedure

Simple recommendations that will allow you to get accurate and reliable results:

  • abstain from sexual intercourse and masturbation at least 3 days before the exam, but no more than 7 days before;
  • 7 days before the biospecimen is submitted, refuse to drink all types of alcohol;
  • Quit smoking 3 hours before the biospecimen is submitted;
  • do not take hot baths, visit saunas or steam rooms 10 days before the delivery of the biomaterial;
  • If you take medications on a regular basis, especially if they include sedatives and antidepressants, please inform the IVMED clinic medical staff before you decide to take a spermogram. This is necessary to record this information in the study protocol.

Features and interesting facts

  • Masturbation is the ideal method for collecting a semen analysis.
  • It is not recommended to collect semen during interrupted sexual intercourse.
  • It is not recommended to collect semen from a used condom. Close contact with the latex material and lubricant in the condom can reduce the active functions of sperm and cause inaccurate diagnostic results.
  • It is recommended to keep the temperature in the range of 18 -25 C during all stages of sample collection and storage.
  • If the temperature changes or goes beyond the critical values, this will also affect the active functions of the sperm and cause inaccurate test results.
  • The World Health Organization and IVMED clinic specialists advise to donate sperm in the clinic so that diagnostics of the donated material can begin immediately after the ejaculate is donated.

 

Additional information:

  • One ejaculate sample can be used (with a sufficient volume) for all types of semen analysis at one time! That is, you can get the results of several tests at once.
  • For a comprehensive assessment of the male factor of infertility, we recommend obtaining the results of other ejaculate tests.
