1. When paying for services on the website https://ivmed.ua, the User – an individual, a subject of personal data, provides Rodynne Dzherelo LLC with his unconditional consent to the collection and processing of personal data.
2. I, as the User of the website https://ivmed.ua, give my consent to Rodynne Dzherelo LLC to process, use and store my personal data in the files and/or through the personal data base owned by Rodynne Dzherelo LLC (hereinafter referred to as the Owner) and located at the following address: Kyiv, Ukraine. Kyiv, ul. 2-B, Antonov Aviation Design Bureau.
3. When placing an order on the website https://ivmed.ua, I confirm that I have been informed and agree to the following:
-The Owner processes the User’s personal data by any means in order to
proper provision of services to the User (identification, authentication, authorization,
depersonalization, password recovery, sending information materials with the consent of the User, responding to requests and letters from the User, receiving applications for the purchase of the service, leaving feedback on the quality of the Owner’s service, information about promotions, sweepstakes, other information about the Owner’s activities in order to conduct successful and secure order and payment transactions);
-While using the Website, the Owner collects and processes the User’s personal data related to identification, authentication, authorization, depersonalization, password recovery, sending information materials with the User’s consent, responding to the User’s requests and letters, receiving applications for the purchase of services, leaving feedback on the quality of the Owner’s service, for registration in the Owner’s discount program, other information about the Owner’s activities, and the implementation of the services offered;

registration form “Leave a request”, “Ask a question to a reproductive specialist”, “Send a request”, payment for “Skype consultation of an obstetrician-gynecologist and reproductive specialist”, I voluntarily agree to provide the above personal data about myself for public access to an unlimited number of persons within the framework of using the Personal Database “Individuals participating in the economic activities of Rodynne Dzherelo LLC”.

materials by age, gender, other data; for statistical research; and in other ways.
-The owner of the database does not inform the personal data subject about the transfer of his or her personal data in cases provided for by the Law of Ukraine “On Personal Data Protection”.
4. The User has all the rights to protect his/her personal data provided for by the current
the legislation of Ukraine, in particular, the Law of Ukraine “On Personal Data Protection”, namely:
– to know about the sources of collection, location of their personal data, the purpose of their processing, location or place of residence (stay) of the owner or manager of personal data or to give a corresponding order to obtain this information to persons authorized by him/her, except in cases established by law;
– receive information about the conditions for granting access to personal data, including information about third parties to whom his or her personal data is transferred;
– to access their personal data;
– to receive a response on whether his/her personal data is processed no later than thirty calendar days from the date of receipt of the request, except as provided by law, and to receive the content of such personal data;
– submit a reasoned request to the personal data controller with an objection to
processing of their personal data;
– to make a reasoned request to change or destroy their personal data by any owner and administrator of personal data if this data is processed illegally or is unreliable;
– to protect their personal data from unlawful processing and accidental loss, destruction, damage due to intentional concealment, failure to provide or untimely provision, as well as to protect against the provision of information that is inaccurate or discrediting the honor, dignity and business reputation of an individual;
– file a complaint against the processing of their personal data with the Commissioner or a court;

7. This agreement is provided in compliance with the requirements of Articles 7, 8 and 11 of the Law of Ukraine “On Protection of
Personal Data” and is valid without any territorial restrictions.
8. I hereby confirm that I have received a notice of the Personal Data Owner, the composition and content of the collected personal data, as well as information about my rights under the Law of Ukraine “On Personal Data Protection” to fulfill the specified purpose and the possibility of transferring my personal data to third parties at the discretion of the Owner and/or the database manager.
9.Content of my rights as a personal data subject in accordance with the Law of Ukraine “On Protection of
of personal data” has been communicated to me and I understand it.
10.The said agreement shall be valid indefinitely until the date of its revocation by me by sending to the address of Rodynne Dzherelo LLC a written notice of the said revocation in any form, unless otherwise provided by the current legislation of Ukraine.
11.Rodynne Dzherelo LLC reserves the right to make changes to this
position. After the changes are made, such changes will take effect immediately after they are posted on the official website of Rodynne Dzherelo LLC
